• Known serious hypersensitivity to alirocumab or excipients
• Pregnancy (limited data; lipid lowering generally deferred in pregnancy)

• Statins
• Other lipid-lowering agents (ezetimibe)

• Concurrent simvastatin >20 mg or pravastatin >40 mg (bempedoic acid raises their levels)
• Active gout flare without management
• History of tendon rupture related to similar agents

• Corticosteroids / fluoroquinolones
