Approved regimen (Egrifta, HIV-associated lipodystrophy only): 2 mg once daily by subcutaneous injection. This is a prescription drug — the dose, indication and monitoring are set by a prescriber. The cognition RCT used a lower 1 mg/day. There is NO validated dose for off-label 'anti-aging' or physique use, and any such use is outside the evidence base.

• Personal or family history of medullary thyroid carcinoma (MTC)
• Multiple Endocrine Neoplasia syndrome type 2 (MEN2)
• Prior serious hypersensitivity to semaglutide or any GLP-1 receptor agonist

• Insulin and sulfonylureas
• Oral medications (general)
• Other GLP-1 receptor agonists

• Active malignancy — tesamorelin raises IGF-1, a theoretical proliferative/neoplasia concern; it is contraindicated in active cancer
• Pregnancy — contraindicated; GH/IGF-1 stimulation is inappropriate in pregnancy
• Disruption of the hypothalamic-pituitary axis (hypophysectomy, pituitary tumor/surgery/radiation, head irradiation) per the approved label

• Insulin / glucose-lowering drugs
• Corticosteroids / cortisone-replacement
• Drugs metabolized by CYP3A (e.g. some statins, certain antiretrovirals)
