Einfach gesagt: In 458 men randomised across 45 European sites, target area hair count at week 24 rose 20.2 hairs with topical finasteride against 6.7 with placebo (P < 0.001), and was numerically similar between topical and oral finasteride.
The pivotal trial: phase III, double-blind, DOUBLE-DUMMY, so it compares topical against placebo AND against oral finasteride in the same study.
Primary endpoint met — target area hair count +20.2 vs +6.7 hairs at 24 weeks (P < 0.001).
Topical and oral were NUMERICALLY SIMILAR on hair count. The abstract does not claim topical is better at growing hair; the argument for it is exposure, not efficacy.

⚠️ THE CAVEAT THAT MATTERS: serum DHT fell 60-70%. "Topical avoids systemic exposure" is not what this study shows at the higher doses.
Dose changes the trade. At 100 and 200 µL, scalp DHT fell 47-52% while serum DHT fell only 24-26%; at 300 and 400 µL serum DHT fell 44-48%.
So the systemic-sparing claim is DOSE-DEPENDENT, and the doses that spare serum DHT are the lower ones.

Within-patient bilateral design: each woman is her own control, which is unusually strong for a small study.
Hair density fell from about 19.7 to 8 on the finasteride side and 19.1 to 9 on placebo, after up to three IPL plus radiofrequency sessions every four weeks and a further session at six months.
