11 RCTs, 729 patients with IBS or other functional bowel disorders
Inulin-type fructans WORSENED flatulence severity (SMD 0.85; 95% CI 0.23, 1.47; P=0.007), while non-inulin-type fructan prebiotics improved it (SMD -0.34)
No difference in global response between prebiotics and placebo (OR 0.62; 95% CI 0.07, 5.69; P=0.67)
2Bifidobacterium abundanceMeta-Analysen=2,099 · very large study2018
Einfach gesagt: Fructans and galacto-oligosaccharides led to significantly greater abundance of both Bifidobacterium and Lactobacillus — with no effect on alpha-diversity.

64 studies, 2099 participants, healthy adults
Fibre raised Bifidobacterium spp. (SMD 0.64; 95% CI 0.42, 0.86; P<0.00001) and Lactobacillus spp. (SMD 0.22; 0.03, 0.41; P=0.02)
Fructans — the family short-chain FOS belongs to — were one of only two subgroups significantly raising both genera (P<0.00001)
Yang J et al. · The Journal of clinical investigation (2024)
Body composition, commonly used scales, biochemical markers, intestinal microbiota, and metabolites were examined in 3 groups of older individuals (nonfrail, prefrail, and frail).
Additionally, it stimulated an elevation in gut probiotic count and induced alterations in microbial metabolite expression levels as well as corresponding metabolic pathways.CONCLUSIONSThe findings suggest a potential link between changes in the gut microbiota and frailty in older adults.
Einfach gesagt: Further research is needed to determine if anti-inflammatory effects depend on the specific pre-, pro-, and synbiotic preparations, health status, and biomarkers studied.
Kadia BM et al. · Nutrients (2024)
In six studies of healthy children (n = 1552), one reported that fructo-oligosaccharides added to infant formula significantly reduced pro-inflammatory biomarkers, and one study of a single-strain probiotic reported both anti- and pro-inflammatory effects.
Significantly reduced inflammatory biomarkers were reported in 7/10 studies of atopic/allergic conditions, 3/5 studies of autoimmune diseases, 1/2 studies of preterm infants, 1 study of overweight/obesity, 2/2 studies of severe illness, and 2/3 studies of other diseases.
However, only one or two of several biomarkers were often improved; increased pro-inflammatory biomarkers occurred in five of these studies, and a probiotic increased inflammatory biomarkers in a study of newborns with congenital heart disease.
Einfach gesagt: More prospective, controlled studies should be conducted to reveal specific details regarding the fiber type, dosage, and duration for optimal intervention.
Stachowska E et al. · Nutrients (2020)
These effects suggest the potential benefits of fiber consumption for NAFLD populations.

Bifidobacterium spp. counts increased significantly with FOS (WMD 0.579; 95% CI 0.444-0.714) versus control
Larger effect in adults (0.861) than infants (0.458)
Larger effect beyond 4 weeks (0.841) than at or under 4 weeks (0.532), and at doses above 5 g (1.116) than at or under 5 g (0.521)
Einfach gesagt: At an identical 10 g/day, short-chain fructooligosaccharides were bifidogenic (P=0.008) while long-chain inulin was NOT — the cleanest evidence that FOS and chicory inulin are not interchangeable.
Bouhnik Y et al. · Am J Clin Nutr (2004)

200 healthy volunteers total, but the head-to-head screening phase (phase 1) randomised only 64 of them — 8 groups of 8 — to seven non-digestible carbohydrates or placebo at 10 g/d for 7 days. The remaining 136 entered a phase 2 dose-response restricted to the carbohydrates already shown bifidogenic in phase 1, so long-chain inulin was never tested beyond n=8
Bifidogenic: short-chain fructooligosaccharides (P=0.008), soybean oligosaccharides (P=0.006), galactooligosaccharides (P<0.0001) and type III resistant starch (P=0.02)
NOT bifidogenic: lactulose, LONG-CHAIN INULIN, and isomaltooligosaccharides
9Glycaemic metabolism and gut microbiota compositionRCTn=131 · medium study2025
Einfach gesagt: Head to head, inulin improved glycaemic metabolism in overweight and obese participants while FOS did not improve glycaemic metrics in either group.
Li J et al. · BMC Med (2025)
131 adults randomised to inulin (n=44), FOS (n=43) or placebo (n=44) for 4 weeks
Inulin significantly reduced 1-hour (Cohen's d 0.71, P=0.041) and 2-hour (d 0.73, P=0.028) OGTT glucose, increased fasting insulin (d 0.70, P=0.008) and lowered homocysteine (d 0.76, P=0.014) in overweight/obese individuals
FOS significantly decreased homocysteine (d 0.72, P=0.023) but did NOT improve glycaemic metrics in either the overweight/obese or the healthy group

Einfach gesagt: The use of FOS to improve the absorption of minerals and trace elements seems to be beneficial with evidence corroborating both in human and animal studies.
Costa GT et al. · Clinical nutrition ESPEN (2021)
The use of FOS to improve the absorption of minerals and trace elements seems to be beneficial with evidence corroborating both in human and animal studies.
However, the literature lacks articles exploring the daily dose and duration for FOS benefits, as well as long-term side effects in healthy or unhealthy subjects.
Future research should focus on addressing the extent of the functional effect of this fiber and identifying the impact on overall health.
Einfach gesagt: Studies suggest that FOS supplementation is positively associated with an anti-inflammatory and antioxidant effect, thus enhancing the gut immune system, which may be beneficial for the host's health.

Einfach gesagt: Large-scale, long-term trials of 1-kestose interventions for PDAC are thus warranted to improve the prognosis of PDAC.
Nakaoka K et al. · Nutrients (2024)
In the 1-kestose administration group, the cancer marker CA19-9 significantly decreased, and there was a reduction in the neutrophil-to-lymphocyte ratio (NLR).
Additionally, Escherichia coli, which typically increases in PDAC, significantly decreased in the 1-kestose group.
There was also suppression of the reduction of albumin levels and of an increase in C-reactive protein.

Einfach gesagt: Although the alterations in gut microbiota and peripheral immune cell composition in those receiving FOS are intriguing, additional studies are required to investigate the use of prebiotics in HCT recipients.
Andermann TM et al. · Transplantation and cellular therapy (2021)
FOS is well tolerated at 10 g/d in patients undergoing reduced-intensity allo-HCT.
Although the alterations in gut microbiota and peripheral immune cell composition in those receiving FOS are intriguing, additional studies are required to investigate the use of prebiotics in HCT recipients.
Einfach gesagt: Prebiotics, particularly FOS and germinated barley foodstuff, show potential as effective and safe dietary supplements for induction and maintenance of remission in UC, respectively.
Limketkai BN et al. · Inflammatory bowel diseases (2025)
For induction of clinical remission in ulcerative colitis (UC), the fructooligosaccharide (FOS) kestose was effective (relative risk, 2.75, 95% confidence interval, 1.05-7.20; n = 40), but oligofructose-enriched inulin (OF-IN) and lactulose were not.
For maintenance of remission in UC, germinated barley foodstuff trended toward preventing clinical relapse (relative risk, 0.40; 95% confidence interval, 0.15-1.03; n = 59), but OF-IN, oat bran, and Plantago ovata did not.
Prebiotics, particularly FOS and germinated barley foodstuff, show potential as effective and safe dietary supplements for induction and maintenance of remission in UC, respectively.
17Overall response to treatment in IBSRCTn=96 · small study2000
Einfach gesagt: The only dedicated IBS trial was negative: after 4-6 weeks of treatment, IBS symptoms improved MORE in the placebo group than in the fructooligosaccharide group.
Olesen M, Gudmand-Hoyer E · Am J Clin Nutr (2000)
Multicentre, randomised, double-blind, placebo-controlled parallel trial at 24 sites; 20 g FOS/d (n=52) or placebo (n=46) for 12 weeks after a 2-week run-in
After 4-6 weeks of treatment, symptoms improved MORE in the placebo group than in the FOS group
At completion there were NO significant differences: symptoms improved in 58% on FOS versus 65% on placebo, and worsened in 8% versus 13%
Einfach gesagt: Based on these results, there is fair evidence that prebiotics and substances with prebiotic properties may improve metabolic and inflammatory biomarkers related to T2DM in women aged 18 years at least.
Colantonio AG et al. · Journal of the Academy of Nutrition and Dietetics (2020)
Based on these results, there is fair evidence that prebiotics and substances with prebiotic properties may improve metabolic and inflammatory biomarkers related to T2DM in women aged 18 years at least.
Interventions with resistant starch, resistant dextrin, and oligofructose-enriched inulin exhibited the strongest evidence for improvements due to the quantity of publications and quality grades.
Other prebiotics and substances with prebiotic properties show promise but the number of studies is few.
Einfach gesagt: Overall, the ingestion of a single dose of OI after an exercise session appears to induce subtle reductions in appetite, although the impact of these changes on acute and prolonged EI remains unclear.
Hamilton CC et al. · Nutrients (2023)
In a randomized crossover study, following the completion of a 45 min (65-70% VO2peak) evening exercise session, participants (BMI: 26.9 ± 3.5 kg/m2, Age: 26.8 ± 6.7 yrs) received one of two beverages: (1) sweetened milk (SM) or (2) sweetened milk + 20 g OI (SM+OI).
Perceived measures of hunger were reduced in SM+OI relative to SM (p = 0.009).
EI during the ad libitum breakfast in the morning postexercise tended to be lower in SM+OI (p = 0.087, d = 0.31).
