Fibre raised Bifidobacterium spp. overall (SMD 0.64; 95% CI 0.42, 0.86; P<0.00001) and Lactobacillus spp. (SMD 0.22; 0.03, 0.41; P=0.02)
In subgroup analysis, fructans and galacto-oligosaccharides specifically produced significantly greater abundance of BOTH genera (P<0.00001 and P=0.002)
NO effect on alpha-diversity, on other prespecified bacteria, or on short-chain fatty acids other than butyrate
Einfach gesagt: In conclusion, GOS seems to be a solution to benefit adults with a low stool frequency and middle-aged adults with self-reported constipation.
Schoemaker MH et al. · Nutrients (2022)
Constipation is a major issue for 10-20% of the global population.
Subjects (94% females, aged: 18-59 years) received either 11 g or 5.5 g of BiotisTM GOS, or a control product, once daily for three weeks.
A clear dose-response of GOS was seen on fecal Bifidobacterium, and 11 g of GOS significantly increased Anaerostipes hadrus.
Einfach gesagt: Treatment with RP-G28 also improved milk/dairy consumption and quality of life and was safe and well tolerated.
Chey W et al. · Nutrients (2020)
Results: For the primary endpoint, 40% in the RP-G28 groups reported a ≥4-point reduction or no symptoms on LI symptom composite score compared to 26% with placebo (P = 0.016).
Treatment with RP-G28 also led to significantly higher levels of milk and dairy intake and significant improvements in global assessments compared to placebo.
RP-G28 but not placebo led to significant increases in five Bifidobacterium taxa.
Einfach gesagt: This review did not find any evidence for the use of prebiotics in people with CF.
Williams NC et al. · The Cochrane database of systematic reviews (2023)
This review did not find any evidence for the use of prebiotics in people with CF.
Until such evidence is available, it is reasonable for clinicians to follow any local guidelines and to discuss the use of dietary prebiotics with their patients.
Large and robust RCTs assessing the dietary prebiotics of inulin or galacto-oligosaccharides or fructo-oligosaccharides, or any combination of these, are needed.
Einfach gesagt: Adding prebiotics to iron-fortified infant cereal increases iron absorption and reduces the adverse effects of iron on the gut microbiome and inflammation in Kenyan infants.
Mikulic N et al. · The American journal of clinical nutrition (2024)
Median (25th-75th percentiles) FIAs from meals before intervention were as follows: 16.3% (8.0%-27.6%) without prebiotics compared with 20.5% (10.4%-33.4%) with prebiotics (Cohen d = 0.53; P < 0.001).
FIA from the meal consumed without prebiotics after intervention was 22.9% (8.5%-32.4%), 41% higher than from the meal without prebiotics before intervention (Cohen d = 0.36; P = 0.002).
FIA from the meal consumed with prebiotics after intervention was 26.0% (12.2%-36.1%), 60% higher than from the meal without prebiotics before intervention (Cohen d = 0.45; P = 0.007).

69 Rome III IBS patients randomised to sham diet plus placebo, low-FODMAP diet plus placebo, or low-FODMAP diet plus 1.4 g/d B-GOS for 4 weeks
Adequate symptom relief was higher with low-FODMAP diet plus B-GOS (16/24, 67%) than control (7/23, 30%) — OR 4.6 (95% CI 1.3-15.6; P=0.015)
Bifidobacterium concentrations were NOT different between the low-FODMAP diet with and without B-GOS, and were LOWER in the B-GOS arm (9.49 log10 cells/g) than control (9.77; P=0.018)
Einfach gesagt: We found little evidence that maternal prebiotics supplementation during pregnancy and lactation reduces the risk of medically diagnosed infant eczema by age 1 year in infants who are at hereditary risk of allergic disease.
Palmer DJ et al. · The Journal of allergy and clinical immunology (2025)
There was no significant difference between groups in the percentage of infants with medically diagnosed eczema by age 1 year (prebiotics 31.5% [103 of 327 infants] vs placebo 32.6% [105 of 322 infants]; adjusted relative risk, 0.98; 95% CI, 0.77-1.23; P = .84).
We found little evidence that maternal prebiotics supplementation during pregnancy and lactation reduces the risk of medically diagnosed infant eczema by age 1 year in infants who are at hereditary risk of allergic disease.
RESULTS: Between June 2016 and November 2021, 329 women were assigned to the prebiotic group (50.4%), and 323 (49.5%) were assigned to the placebo group.
Overall, 288 of 652 women (44.2%) were classified as overweight/obese prior to pregnancy (BMI ≥ 25 kg/m2).
The distribution was balanced, with 146/329 (44.4%) randomised to the prebiotics group and 142/323 (44.0%) in the placebo group.
Einfach gesagt: Conclusions: There were no group benefits of B-GOS supplementation compared with placebo, although the individual variation may warrant further research in larger sample sizes and longer-duration studies.
Gough LA et al. · Nutrients (2025)
A 20 min run at 80% VO2max was completed, with measures for GI distress and Competitive State Anxiety Inventory-2 questionnaire (CSAI-2) pre- and post-exercise.
Results: There were no significant differences in VO2max (p = 0.437), GI discomfort (p = 0.227), or CSAI-2 (p = 0.739-0.954) from pre- to post-exercise at any time point or between conditions.
Over the 12 weeks there were no significant differences between B-GOS and placebo in IPAQ-7 (p = 0.144-0.723), REST-Q (p = 0.282-0.954), WEMWBS (B-GOS pre = 51 ± 10, post = 53 ± 7; PLA pre = 51 ± 4, post 54; p = 0.862), or sleep (p = 0.065-0.992).
Einfach gesagt: Certain probiotics may enhance mood, but their influence on the gastrointestinal microbiota requires further investigation.
Taylor AM et al. · Nutritional neuroscience (2020)
Probiotic consumption improved psychological or biological measures of depression, anxiety, or stress in individuals predisposed to a mood disorder.
Probiotics suppressed biological markers of stress in healthy individuals in a strain-dependent manner.Discussion: High-quality diets, prebiotics, and probiotics may beneficially affect mood.
Habitual diets rich in dietary fiber and omega-3-polyunsaturated fatty acids may be linked to reduced risk of developing symptoms of depression, anxiety, and stress; however, additional studies are necessary.
Einfach gesagt: This review aims to explore the effects of different prebiotics on T2DM, their impact on gut microbiota composition, and the potential for personalized dietary interventions to optimize diabetes management and improve overall health outcomes.
Iatcu OC et al. · Nutrients (2024)
These prebiotics can directly impact blood sugar levels while promoting the growth of beneficial bacteria, thus enhancing glycemic control.
Studies have shown that T2DM patients often exhibit a decrease in beneficial butyrate-producing bacteria, like Roseburia and Faecalibacterium, and an increase in harmful bacteria, such as Escherichia and Prevotella.
This review aims to explore the effects of different prebiotics on T2DM, their impact on gut microbiota composition, and the potential for personalized dietary interventions to optimize diabetes management and improve overall health outcomes.
Einfach gesagt: DP reduced salivary cortisol and stimulated Bifidobacterium, which possibly is important for sleep improvement.
Schaafsma A et al. · Nutrients (2021)
Compared to placebo (skimmed milk), PSQI was only lower at day 14 in the 2nd intervention period in intention-to-treat (ITT) (p = 0.017; n = 69) and per-protocol (PP) (p = 0.038; n = 64) analyses.
Post-hoc analysis (modified-PP: n=47, with baseline PSQI ≥ 9, and endline day 14), however, showed a decrease in PSQI (-1.60 ± 2.53; p = 0.034).
Early morning saliva cortisol decreased versus placebo (p = 0.045).
Einfach gesagt: Instead the rate and ratio of SCFA production should be evaluated in parallel.
Harris HC et al. · Critical reviews in food science and nutrition (2021)
Rhamnose ranked highest for propionate production.
The lowest SCFA production was observed for kiwi fiber, polydextrose, and cellulose.
This review demonstrates that choosing a substrate to selectively enhance a specific SCFA is difficult, and the molar proportion of each SCFA produced by individual substrates may be misleading.
Einfach gesagt: A 168-day dietary prebiotic intervention reduced the duration of upper respiratory symptoms and reduced the incidence and severity of gastrointestinal symptoms in elite rugby union players.
Parker C et al. · European journal of sport science (2023)
The prebiotic group experienced a 2-day reduction in upper respiratory symptom duration (P = 0.045).
Gastrointestinal symptom severity and incidence were lower in the prebiotic group compared to the placebo group (P < 0.001, P = 0.041) respectively.
Salivary immunoglobulin A secretion rate was 42% greater in the prebiotic group compared to the placebo group at day 168 (P = 0.004), no differences in CRP and TNF-α were found (P > 0.05).
Einfach gesagt: This suggests that Bifidobacterium increase via GOS supplementation may help improve the gut microbiota composition by altering the desire for specific types of carbohydrates and boosting Bifidobacterium availability when fiber intake is below recommended levels, without compromising appetite for fiber from food.
Johnstone N et al. · Nutrients (2021)
Food diaries and stool samples were collected prior to and following 28 days of supplement consumption.
It was found that four weeks of GOS supplementation influenced macronutrient intake, as evident by reduced carbohydrate and sugars and increased fats intake.
Further analysis showed that the reduction in carbohydrates was predicted by increasing abundances of Bifidobacterium in the GOS group in comparison to the placebo group.
Einfach gesagt: Further clinical investigation of the effect of micronutrients and macronutrients on the microbiome and metabolome is warranted, along with understanding how this influences host health.
Yang Q et al. · Nutrients (2020)
Current research shows consistent clinical evidence that dietary fibers, including arabinoxylans, galacto-oligosaccharides, inulin, and oligofructose, promote a range of beneficial bacteria and suppress potentially detrimental species.
The preclinical evidence suggests that both the quantity and type of fat modulate both beneficial and potentially detrimental microbes, as well as the Firmicutes/Bacteroides ratio in the gut.
Clinical and preclinical studies suggest that the type and amount of proteins in the diet has substantial and differential effects on the gut microbiota.
Einfach gesagt: We found no evidence that maternal intervention with prebiotics protects against AD at 1 year of age in infants at risk of allergic diseases.
Barbarot S et al. · The British journal of dermatology (2026)
Of 376 pregnant women included in the trial, prebiotic supplementation did not prevent AD at 1 year (intention-to-treat population odds ratio 1.01, 95% confidence interval 0.59-1.74; P = 0.97) or reduce disease severity in their children.
We found no evidence that maternal intervention with prebiotics protects against AD at 1 year of age in infants at risk of allergic diseases.
