A prenatal is really one exceptional intervention bundled with a dozen unproven ones.
Periconceptional folic acid cuts neural tube defect risk by roughly 70% — Cochrane rates this HIGH-quality evidence (RR 0.31), backed by two landmark randomised trials and a community trial in nearly 250,000 women where the most adherent group saw an 85% reduction.
Crucially, the neural tube closes in the first weeks of gestation, usually before a woman knows she is pregnant, so this only works if started BEFORE conception. That single timing fact is the most actionable thing on this page. The rest of the formula is weaker than the marketing suggests.
In the 1991 MRC trial, the arm receiving seven other vitamins showed no significant protection, and Cochrane's own analysis found the folate effect unchanged whether folic acid was given alone or with other micronutrients.
Multiple-micronutrient supplements do reduce low birthweight (RR 0.88, high certainty) versus iron and folic acid alone, but 19 of 20 contributing trials came from low- and middle-income countries, and the one UK trial found little or no effect.
There is no mortality benefit at high certainty — stillbirth, perinatal and neonatal mortality all sit at or near RR 1.00.
The popular 'DHA for baby's brain' claim is not supported: Cochrane found very few differences in child cognition, IQ, vision, language or behaviour, and the largest DHA trial was null for early preterm birth.
Product quality is a real issue too: virtually all prenatals exceed the recommended intake for at least one nutrient, and analytical testing predicts iodine, chromium and potassium overages above 20% of label in most non-prescription products.
Folate supports the one-carbon metabolism needed for DNA synthesis and methylation during the rapid cell division of early neural tube formation. The evidence for specificity is strong: other vitamins in the same trial did nothing, and the effect scales with adherence. The pre-conception timing requirement is a biological inference — the neural tube closes in the first weeks, and every trial that showed benefit started supplementation before conception — but no trial has randomised pre-conception versus post-conception start.
Iodine is the substrate for thyroid hormones, which regulate fetal growth and brain development, and requirements rise substantially in pregnancy. The mechanism is sound and low maternal iodine tracks with lower child verbal IQ — but supplementation trials have not confirmed the cognitive benefit.
Pregnancy expands maternal plasma volume and adds fetal demand. Cochrane grades the evidence that daily iron reduces maternal anaemia as LOW certainty — it MAY reduce it — with only iron-deficiency anaemia at term reaching moderate certainty. The depletion-repletion mechanism is plausible and partly supported, not demonstrated.
Omega-3 appears to shift the whole gestational-age distribution later, which reduces preterm birth but probably also increases post-term pregnancy — Cochrane grades the prolonged-gestation increase as moderate certainty ('probably increased'). A coherent whole-distribution signature rather than a selective preterm effect.

Folic acid should be started before conception. Because the neural tube closes before most pregnancies are recognised, waiting for a positive test misses the window.
High-dose folic acid — 4 mg/day in US guidance, 5 mg/day in UK, Canadian and most European guidance — started about 3 months before conception and continued through 12 weeks. Recognised indications are broader than most people realise: a previous NTD-affected pregnancy (recurrence evidence is strong, RR 0.34); YOU have an NTD; your PARTNER has an NTD or has had an NTD-affected child; a first- or second-degree relative with an NTD; taking a folate-antagonist medicine (methotrexate, carbamazepine, valproate, phenytoin, trimethoprim, sulfasalazine); pre-gestational (not gestational) type 1 or 2 diabetes; and blood disorders needing extra folate such as sickle cell disease or thalassaemia. Reaching 4-5 mg needs a separate prescription-strength product — do NOT get there by taking extra prenatal tablets, which would also multiply your vitamin A, iron and iodine into unsafe territory. Confirm the plan with your clinician.
These drugs raise neural-tube-defect risk and are a recognised indication for high-dose folic acid — 4 mg (4,000 mcg) daily, from 3 months preconception through 12 weeks. This must be arranged with the prescriber who manages your epilepsy, not started independently: folic acid can lower phenytoin levels and affect seizure control, and switching or stopping an antiepileptic in pregnancy carries its own serious risks.
Diabetes present before pregnancy raises neural-tube-defect risk and is a recognised indication for high-dose folic acid — 4 mg (4,000 mcg) daily, from 3 months preconception through 12 weeks. Glycaemic control before conception matters at least as much as the supplement; both should be planned with your clinician. Note this means diabetes that predates the pregnancy, not gestational diabetes.
Malabsorptive procedures (gastric bypass, duodenal switch) impair absorption of folate, iron, B12, calcium and the fat-soluble vitamins, so a standard prenatal may not be sufficient and deficiencies are common. Requirements are individualised and need clinician-supervised monitoring of nutrient levels rather than a self-selected off-the-shelf product.
Store out of reach. Iron was identified as a major cause of unintentional poisoning death in children under 6 in the era before unit-dose packaging; after the 1997 US packaging regulation, deaths in that age group fell from 29 in the previous 10 years to 1 in the following 5 years. The hazard is much reduced, not eliminated.
Iodine content varies and is frequently higher than the label states. Discuss with your clinician.
Choline has a pregnancy Adequate Intake of 450 mg/day (NASEM; ACOG discusses it), and it is the nutrient most often absent or heavily under-dosed in prenatals — worth checking the label specifically. For iodine, the American Thyroid Association, American Academy of Pediatrics and Endocrine Society all recommend a supplement supplying 150 mcg/day in preconception, pregnancy and lactation; ACOG has notably not endorsed this, and supplementation trials have been null, so the honest position is that major bodies disagree. Given analytical testing finds iodine frequently over label in non-prescription prenatals, the aim is roughly 150 mcg — not zero, and not several times that.
Obesity is associated with higher neural-tube-defect risk, but guidance disagrees on whether it warrants high-dose folic acid. NICE reviewed the evidence and found none supporting high-dose for a raised BMI alone, concluding 400 mcg is sufficient without other risk factors; SOGC recommends 5 mg; ACOG lists it as a risk factor while noting obesity-associated NTDs may be folate-resistant. Take the standard dose unless you have another listed risk factor, and raise it with your clinician.
Prescription retinoids are established human teratogens in their own right. Adding a prenatal containing preformed retinol pushes total vitamin A exposure higher still, against an official Upper Intake Level of 10,000 IU/day (3,000 mcg RAE) whose designated critical effect is teratogenicity. Stacking retinol sources in pregnancy has no upside and should be avoided outright.
These drugs are folate ANTAGONISTS — they deplete folate, and taking them early in pregnancy raises neural-tube-defect risk (the adjusted odds ratio for neural tube defects with first/second-month trimethoprim exposure is about 4.8). The guidance therefore points the opposite way to what people often assume: if you take one of these and are trying to conceive or are in the first 12 weeks, clinicians usually prescribe HIGHER-dose folic acid (4-5 mg/day), not less. Do not reduce or stop folic acid because you are on one of these medicines. Methotrexate is a separate and more serious case: it is an established teratogen and abortifacient, contraindicated in pregnancy, and needs a clinician-managed plan before conception rather than a self-selected prenatal. Never start, stop or change either the medicine or your folic acid dose without your prescriber.
This runs in both directions. Folic acid supplementation can lower phenytoin serum concentrations and precipitate loss of seizure control, so levels need monitoring. Conversely, these drugs raise neural-tube-defect risk and are themselves an indication for high-dose (4 mg/day) folic acid — see special populations. Both sides require prescriber supervision; do not self-manage either the folate dose or the antiepileptic.
Iron and calcium in prenatals are widely reported to reduce thyroid hormone absorption when taken together; separate the doses by several hours and discuss with your clinician.

A first-line antihypertensive IN pregnancy, and iron-containing prenatals are near-universal — so this pairing is common. Ferrous sulfate chelates methyldopa and cut its bioavailability by about 83% when taken together (still ~55% reduced at 1 hour apart, ~42% at 2 hours), with documented loss of blood-pressure control that reversed within a week of stopping the iron. Separate them by as long as your clinician advises and monitor your blood pressure.
US perinatal HIV guidelines state dolutegravir should not be taken within 2 hours of anything containing iron or calcium, explicitly including prenatal vitamins. Fasted co-administration cut dolutegravir exposure by 54-57% with iron and 37-39% with calcium — enough to risk virologic failure. Separate the prenatal from the antiretroviral by at least 2 hours, or take it with a meal exactly as your HIV clinician directs.

The commonly studied dose of Prenatal Multivitamin is 1 serving daily as labelled. The verified folic acid evidence rests on 400-800 mcg daily started before conception. Individual needs vary — start at the lower end of the range and adjust based on how you respond.
The best time to take Prenatal Multivitamin is with meals. Take it with food. Taking with food aids absorption of fat-soluble vitamins and reduces the nausea and stomach upset that commonly cause women to stop taking iron-containing prenatals.
Prenatal Multivitamin is generally safe at recommended doses, with a few precautions worth noting. The most commonly reported side effects are nausea, constipation, digestive intolerance from iodine. Use caution if any of these apply to you: Products supplying more than 10,000 IU/day (3,000 mcg RAE) of preformed vitamin A (retinol) in pregnancy — this is the established Tolerable Upper Intake Level for preformed vitamin A, set by the IOM/NASEM and reaffirmed by EFSA in 2024, with teratogenicity as the designated critical effect; Concurrent use of a separate high-dose vitamin A supplement or prescription retinoid; Haemochromatosis or iron overload (iron-containing products).
