What the evidence says
Mostly mechanism / observational
Most Tazarotene studies are mechanism or observational rather than RCTs that measure a clinical effect — keep findings provisional.
Most evidence is from high-quality meta-analyses and randomised trials published 2002–2025 with a typical study size of 684 participants.

1RCTCited 27×n=1,614 · large study2020
In plain English: Two identical double-blind, vehicle-controlled 12-week phase 3 studies randomised 1614 subjects aged 9 and older to tazarotene 0.045% lotion or vehicle once daily.
Tanghetti E, et al. · J Drugs Dermatol (2020)
The two identical phase 3 registration trials for the 0.045% lotion — 1614 subjects aged 9 and older.
The paper’s own background states that irritation from the cream, foam and gel formulations has limited tazarotene’s use in practice, which is the stated motivation for this lotion.
Treatment success required a 2-grade EGSS improvement AND clear/almost clear — the stricter endpoint, not "improved".
2Meta-AnalysisCited 7×n=33,472 · very large study2025
In plain English: Across 35 randomized trials and 33,472 participants comparing nine topical agents, the greatest reductions in non-inflammatory, inflammatory and total lesion counts came from adapalene-benzoyl peroxide, clindamycin-benzoyl peroxide and clindamycin-tretinoin.
Stuart B, et al. · J Eur Acad Dermatol Venereol (2025)
THE COUNTER-EVIDENCE for acne: a network meta-analysis of 35 RCTs and 33,472 participants across nine topical agents.
The top performers were all COMBINATIONS — adapalene-BPO (RoM 1.76 noninflammatory), clindamycin-BPO and clindamycin-tretinoin. Tazarotene monotherapy is not among the leaders.
Inclusion required at least 12 weeks of treatment in patients aged 12 and over.
3RCTCited 2×n=684 · large study2020
In plain English: Pooled paediatric data from the two phase 3 trials: at week 12, inflammatory lesions fell 55.6% versus 37.0% on vehicle in ages 10-13 and 53.3% versus 41.2% in ages 14-17 (P<0.01), with no significant differences between the tazarotene-treated age groups.
Eichenfield L, et al. · J Drugs Dermatol (2020)
A pooled paediatric SUBGROUP analysis of the two phase 3 trials, not a separate trial: 136 patients aged 10-13 and 548 aged 14-17.
Inflammatory lesions -55.6% vs -37.0% (10-13) and -53.3% vs -41.2% (14-17); noninflammatory -47.7% vs -28.2% and -52.7% vs -32.9%.
The abstract reports NO significant difference between the two tazarotene-treated age groups — the drug behaved the same across ages, which is the point of the analysis.
4RCTCited 62×n=563 · large study2002
In plain English: At week 24, tazarotene 0.1% cream produced significantly greater treatment success and at least a 1-grade improvement in fine and coarse wrinkling, mottled hyperpigmentation, lentigines, elastosis, pore size, irregular depigmentation and tactile roughness (P<.01) — and 20 of 283 treated patients discontinued for adverse events against 1 of 280 on vehicle.
Phillips TJ, et al. · Arch Dermatol (2002)
The long photodamage trial: 24 weeks double-blind, then a 28-week open-label extension — 12 months in total, 563 patients.
Significantly greater treatment success (≥50% global improvement) plus at least 1-grade improvement in fine and coarse wrinkling, mottled hyperpigmentation, lentigines, elastosis, pore size, irregular depigmentation and tactile roughness (P<.01). Telangiectasia and actinic keratoses were assessed but are not in that list.
Improvement continued with treatment and had NOT plateaued by week 52 — the benefit was still accruing when the study ended.
5RCTCited 30×n=568 · large study2005
In plain English: In 568 patients treated once daily for 24 weeks, tazarotene 0.1% cream was significantly more effective than vehicle on fine wrinkles, mottled hyperpigmentation, lentigines, irregular depigmentation, pore size, elastosis and tactile roughness.
Kang S, et al. · J Am Acad Dermatol (2005)
The confirmatory photodamage trial, 568 patients, 24 weeks.
Significance appeared as early as week 2 on some measures and had NOT plateaued by week 24 — the benefit was still accruing when the trial stopped.
Improvement spanned pigmentation as well as wrinkling, which is why this entry maps to even skin tone as well as wrinkles.
6Meta-AnalysisCited 3×n=3,905 · very large study2025
In plain English: Across 23 RCTs and 3905 participants, tazarotene was most effective for COARSE wrinkles, while isotretinoin ranked highest for fine wrinkles — and tazarotene carried higher adverse-event risk, with isotretinoin and tretinoin judged the most balanced overall.
Wang Y, et al. · Sci Rep (2025)
Bayesian network meta-analysis, 23 RCTs, 3905 participants, comparing topical anti-photoaging agents head to head.
Tazarotene ranked FIRST for coarse wrinkles. That is the specific claim this entry rests on.
⚠️ And it ranked among the WORST on safety: tazarotene and glycolic acid carried higher adverse-event risk, while tretinoin had the most favourable profile.
7RCTCited 36×n=418 · medium study2018
In plain English: In two phase 3 trials (N = 418), halobetasol 0.01% plus tazarotene 0.045% lotion achieved treatment success in 35.8% and 45.3% of subjects at week 8, versus 7.0% and 12.5% on vehicle (P < .001).
Gold LS, et al. · J Am Acad Dermatol (2018)
Plaque psoriasis, studied as a fixed combination of halobetasol propionate 0.01% with tazarotene 0.045% lotion.
Treatment success 35.8%/45.3% vs 7.0%/12.5% at 8 weeks across two phase 3 studies.
Combining a corticosteroid with tazarotene is described in the abstract as a way to optimise efficacy while minimising the safety and tolerability concerns that limit long-term steroid use.
8RCTCited 3×n=2,029 · very large study2022
In plain English: Across five studies totalling 2029 participants, the two phase 1 dermal-safety studies found tazarotene 0.045% lotion had low potential for irritancy or contact dermatitis and did not induce sensitization.
Kircik L, et al. · J Dermatolog Treat (2022)
A pooled analysis of FIVE company-sponsored trials — two phase 1 single-blind dermal-safety studies, one phase 2 and two phase 3 double-blind vehicle-controlled trials. 2029 participants enrolled; the safety population was 1982.
Low irritancy/contact-dermatitis potential and no sensitisation in the phase 1 studies.
It reduced the severity of hyperpigmentation and erythema, and participants preferred it to their previous acne treatments.
9Systematic ReviewCited 77×2019
In plain English: More patients receiving adapalene tolerated their adverse events than those receiving tazarotene (55.4% vs 24.4%; p < 0.0012), and the review separately reported no significant difference in efficacy between tretinoin and tazarotene.
Leyden J, et al. · Am J Clin Dermatol (2019)
⚠️ THE TOLERABILITY COUNTER-EVIDENCE: only 24.4% of tazarotene patients tolerated their adverse events, against 55.4% on adapalene (p < 0.0012).
⚠️ Separately, and against a DIFFERENT comparator: no significant difference in efficacy between tretinoin and tazarotene.
Topical retinoids beat vehicle on IGA by 24.1-28.8% (p < 0.001); a retinoid WITH benzoyl peroxide beat vehicle by 26.1-34.9% against 7-11.8%. These are separate vehicle-controlled comparisons, not a before/after on the same trials.
10RCTCited 9×n=60 · small study2017
In plain English: In 60 patients over 4 weeks, a 50% or greater reduction in total lesion count was reached by 71% on tazarotene plus clindamycin versus 22% on adapalene plus clindamycin (p = 0.0012).
Kumar S, et al. · Clin Drug Investig (2017)
The only direct head-to-head against another topical retinoid in this set, and it favours tazarotene.
71% vs 22% reached a 50% lesion reduction; the difference in total count change was 6.51 (95% CI 1.91-11.09, p = 0.007).
⚠️ Weigh it accordingly: n = 60, OPEN-LABEL, single centre, and only 4 weeks — against the 12-week standard used by every phase 3 trial above.
