Phosphate Metabolism in Health and Disease
Calcified Tissue International (2021) · Review
Phosphorus is a key element in organic molecules involved in essential cellular functions: ATP energy transfer, DNA/RNA, cAMP signaling, and glycerophospholipid membrane integrity.
Dietary Phosphorus Intake and the Kidney
Annual Review of Nutrition (2017) · Review
Phosphorus is abundant in the food supply of developed countries, occurring naturally in protein-rich foods and as an additive in processed foods.
Effects of Excessive Dietary Phosphorus Intake on Bone Health
Current Osteoporosis Reports (2017) · Review
The average phosphorus intake in the USA is well above the recommended dietary allowance.
Based on meta-analysis of 59 RCTs showing 0.6-1.8% BMD increases. Requires vitamin D co-supplementation for optimal effect. Effectiveness plateaus beyond 1200mg daily.
This curve is deliberately flat: supplemental phosphorus only helps when someone is genuinely deficient, and deficiency is rare because the RDA (~700 mg/day) is easily met from food. Beyond meeting the RDA, added phosphorus provides essentially no benefit to people with normal levels while GI side effects (diarrhea) and — with chronic excess — cardiovascular/renal risk climb steeply. The upper intake level is ~4000 mg/day for most adults (3500 mg/day in pregnancy, 3000 mg/day for adults over 70), and much lower in kidney disease.
