Finasteride (topical) Research
Mostly mechanism / observational
8 peer-reviewed studies
What the evidence says
Mostly mechanism / observational
Most Finasteride (topical) studies are mechanism or observational rather than RCTs that measure a clinical effect — keep findings provisional.
Most evidence is from high-quality meta-analyses and randomised trials published 2009–2025 with a typical study size of 77 participants.

1RCTCited 93×n=458 · medium study2022
In plain English: In 458 men randomised across 45 European sites, target area hair count at week 24 rose 20.2 hairs with topical finasteride against 6.7 with placebo (P < 0.001), and was numerically similar between topical and oral finasteride.
The pivotal trial: phase III, double-blind, DOUBLE-DUMMY, so it compares topical against placebo AND against oral finasteride in the same study.
Primary endpoint met — target area hair count +20.2 vs +6.7 hairs at 24 weeks (P < 0.001).
Topical and oral were NUMERICALLY SIMILAR on hair count. The abstract does not claim topical is better at growing hair; the argument for it is exposure, not efficacy.
2RCTCited 50×n=50 · small study2016
In plain English: At the 1 mL dose, scalp DHT fell about 70% with once-daily topical finasteride and about 50% with twice daily or the 1 mg tablet — while SERUM DHT fell 60-70%.
Caserini M, et al. · Int J Clin Pharmacol Ther (2016)
⚠️ THE CAVEAT THAT MATTERS: serum DHT fell 60-70%. "Topical avoids systemic exposure" is not what this study shows at the higher doses.
Dose changes the trade. At 100 and 200 µL, scalp DHT fell 47-52% while serum DHT fell only 24-26%; at 300 and 400 µL serum DHT fell 44-48%.
So the systemic-sparing claim is DOSE-DEPENDENT, and the doses that spare serum DHT are the lower ones.
3Meta-AnalysisCited 1×2025
In plain English: Across 24 eligible trials, the network meta-analysis confirms the efficacy of conventional monotherapies including oral dutasteride, topical and oral minoxidil, and oral and TOPICAL finasteride for male androgenetic alopecia.
Gupta AK, et al. · J Cosmet Dermatol (2025)
The largest synthesis in this set: 24 trials, eight conventional monotherapies and seven over-the-counter alternatives, ranked by Bayesian network meta-analysis.
Topical finasteride is named among the CONFIRMED conventional treatments, alongside oral finasteride, dutasteride and minoxidil.
Primary outcome was change in total hair density at 24 weeks (hairs/cm2), the same horizon as the phase III trial.
4Systematic ReviewCited 88×2018
In plain English: Across seven included articles, all reported reduced hair loss and increased total and terminal hair counts with topical finasteride — and BOTH scalp and plasma DHT fell significantly, with no change in serum testosterone.
Lee SW, et al. · J Drugs Dermatol (2018)
The systematic review, and it is small: seven articles, including case reports and prospective studies alongside RCTs.
⚠️ It confirms the systemic point independently: PLASMA DHT fell significantly, not just scalp DHT.
Serum testosterone did not change in the reviewed studies.
5RCTCited 55×n=45 · small study2009
In plain English: Over six months in 45 men, there were no significant differences between topical 1% finasteride gel and 1 mg oral finasteride in hair thickness, hair count or size of the bald area.
Hajheydari Z, et al. · Indian J Dermatol Venereol Leprol (2009)
A direct head-to-head of gel against tablet, double-blind and double-dummy, six months.
NO significant difference between topical and oral on any of hair thickness, hair count or bald-area size.
The abstract reports "a significant increase in hair counts and terminal hair counts between the two groups" — phrasing that does not clearly separate within-group gain from between-group difference.
6RCTCited 2×n=164 · medium study2024
In plain English: In 164 men over 12 weeks, efficacy was 86.7% with topical finasteride 0.25% plus minoxidil 5% against 69.1% with minoxidil 5% alone (P = 0.006).
Baloch GM, et al. · J Drugs Dermatol (2024)
The combination question, which is how topical finasteride is usually actually used: added to minoxidil rather than instead of it.
Adding topical finasteride raised efficacy from 69.1% to 86.7% (P = 0.006) over 12 weeks.
⚠️ SINGLE-BLIND, single-centre, and 12 weeks is short for a hair endpoint where the phase III used 24.
7RCTCited 7×n=77 · small study2012
In plain English: In 77 women treated with intense pulsed light plus radiofrequency on both sides of the chin, adding twice-daily finasteride 0.5% solution to one side produced a statistically significant reduction in hair density versus placebo solution on the other.
Ekback M, et al. · J Cosmet Laser Ther (2012)
A DIFFERENT indication — unwanted facial hair in women, not scalp hair loss — and the opposite goal: less hair, not more.
Within-patient bilateral design: each woman is her own control, which is unusually strong for a small study.
Hair density fell from about 19.7 to 8 on the finasteride side and 19.1 to 9 on placebo, after up to three IPL plus radiofrequency sessions every four weeks and a further session at six months.
8Systematic ReviewCited 10×2023
In plain English: Of 14 included articles, ten reported a high rate of alopecia recovery in women taking finasteride; the authors conclude that 5 mg oral finasteride daily could be effective in normoandrogenic women, and separately that topical finasteride is more effective than other topical formulas for treating hair loss.
Nowicka D, et al. · Adv Clin Exp Med (2023)
Covers women with female pattern hair loss, a population the male phase III does not speak to at all.
Two conclusions, and both matter: the headline is about ORAL finasteride at 5 mg, but the review also states that topical finasteride is more effective than other topical formulas.
14 articles selected from 380 found; 10 of the 14 reported high recovery rates.
