Black Cohosh and Calcium are generally complementary — many people take them together. Black cohosh (40–80 mg standardized extract/day) can be combined with calcium and vitamin D3 as part of a menopausal health protocol. Monitor liver function with long-term black cohosh use (rare hepatotoxicity reports).
The combination of black cohosh and calcium has been studied for menopausal bone health management. Some RCTs show modest additive benefits for bone marker preservation and quality-of-life outcomes.
Black cohosh acts on serotonergic and dopaminergic pathways to reduce vasomotor symptoms, and has demonstrated selective estrogenic activity in bone tissue. Its triterpene glycosides modulate osteoblast/osteoclast balance. Combined with calcium, black cohosh may support bone health during menopause while concurrently addressing hot flashes.
