In MTHFR variant carriers, switching to or adding methylfolate to a folic acid-containing multivitamin significantly improves folate bioavailability, homocysteine reduction, and neurotransmitter support (dopamine, serotonin synthesis via tetrahydrobiopterin). However, concurrent use means effectively doubling folate intake—monitor total intake.
Beneficial substitution or addition for MTHFR variant carriers. Ideally, choose a multivitamin already containing L-methylfolate rather than combining with folic acid. If combining, ensure total folate equivalents remain below 1000 mcg DFE/day to avoid masking B12 deficiency. Pair with B12 in the multivitamin or standalone methylcobalamin.

Efficacy of B-vitamins and vitamin D therapy in improving depressive and anxiety disorders: a systematic review of randomized controlled trials
Nutritional neuroscience (2023) · Systematic review · n=2256
Systematic Review and Meta-Analysis of L-Methylfolate Augmentation in Depressive Disorders
Pharmacopsychiatry (2022) · Meta analysis · n=6707
Folic acid/methylfolate for the treatment of psychopathology in schizophrenia: a systematic review and meta-analysis
Psychopharmacology (2018) · Meta analysis · n=925
Effect of multivitamin-mineral supplementation versus placebo on cognitive function: results from the clinic subcohort of the COcoa Supplement and Multivitamin Outcomes Study (COSMOS) randomized clinical trial and meta-analysis of 3 cognitive studies within COSMOS
The American Journal of Clinical Nutrition (2024) · Meta analysis · n=5203
Effects of cocoa extract and a multivitamin on cognitive function: A randomized clinical trial
Alzheimer's & Dementia (2023) · Rct · n=2262
Long-term multivitamin supplementation and cognitive function in men: a randomized trial
Annals of Internal Medicine (2013) · Rct · n=5947
Two randomised trials in people whose depression had not responded to an SSRI (PMID 23212058). In the first (148 patients), L-methylfolate at 7.5 mg a day for 30 days, then 15 mg, did no better than placebo. In the second (75 patients), 15 mg a day throughout beat placebo on response and symptom scores, with side effects no different from placebo. A meta-analysis found benefit only for methylfolate at 15 mg once daily added to an SSRI, with all evidence rated low or very low quality (PMID 29442609). The 800 mcg point comes from an open-label, non-randomised trial of 44 patients where adding methylfolate to an antidepressant improved PHQ-9 scores more by day 28 (40% vs 26%), but remission did not differ (PMID 39844190). No trial compared doses head to head, so no minimum or optimal dose is set.

CoQ10, zinc, selenium, carnitine modestly help sperm parameters — but live-birth evidence is weak.
Saffron, EPA omega-3, SAMe have real adjunct evidence — but these aren’t a substitute for care, and some interact dangerously.
On Ozempic/Wegovy/Mounjaro? What actually helps — muscle preservation, GI relief, nutrient gaps (no upsell).
Safe, but no benefit if you’re not filling a gap. Who actually needs one — and who doesn’t.
