BMJ (2017) · Meta analysis · n=11321
25 RCTs, 11.321 Teilnehmer im Alter von 0 bis 95 Jahren. Vitamin D reduzierte akute Atemwegsinfektionen insgesamt (adjustierte OR 0,88, 95-%-KI 0,81 bis 0,96).
Two randomised trials in people whose depression had not responded to an SSRI (PMID 23212058). In the first (148 patients), L-methylfolate at 7.5 mg a day for 30 days, then 15 mg, did no better than placebo. In the second (75 patients), 15 mg a day throughout beat placebo on response and symptom scores, with side effects no different from placebo. A meta-analysis found benefit only for methylfolate at 15 mg once daily added to an SSRI, with all evidence rated low or very low quality (PMID 29442609). The 800 mcg point comes from an open-label, non-randomised trial of 44 patients where adding methylfolate to an antidepressant improved PHQ-9 scores more by day 28 (40% vs 26%), but remission did not differ (PMID 39844190). No trial compared doses head to head, so no minimum or optimal dose is set.
Both points come from a meta-analysis of double-blind cholecalciferol trials in people aged 60 and over (PMID 15886381). Trials at 700 to 800 IU a day cut hip fracture risk by 26% (RR 0.74) and non-vertebral fractures by 23%, plotted at 800 IU. Trials at 400 IU a day showed no benefit (hip fracture RR 1.15), and the WHI trial of 400 IU with 1,000 mg calcium did not significantly reduce hip fracture (PMID 16481635). Most of these trials also gave calcium, and the Cochrane review found vitamin D alone unlikely to prevent fractures while vitamin D with calcium gave a small reduction (PMID 24729336). Doses above 800 IU were not compared, so no optimal dose or ceiling is set.
CoQ10, zinc, selenium, carnitine modestly help sperm parameters — but live-birth evidence is weak.
Saffron, EPA omega-3, SAMe have real adjunct evidence — but these aren’t a substitute for care, and some interact dangerously.
The honest tier list — proven staples vs situational vs mostly marketing. The hub the other guides feed into.
On Ozempic/Wegovy/Mounjaro? What actually helps — muscle preservation, GI relief, nutrient gaps (no upsell).
