The trials behind these points gave 120 or 240 mg of the standardised extract EGb 761 daily to people with dementia for about 6 months (PMID 25506211, 7 trials, 2,625 patients). A meta-analysis of 9 trials found the benefits on cognition, daily activities and global impression mainly at 240 mg (PMID 25114079), and one of mild dementia subgroups found medium to large effects at 240 mg (PMID 39895346). A network meta-analysis of trials lasting over a year ranked 120 mg best for cognition in Alzheimer's disease (PMID 39312316), so 120 mg is plotted as effective, not null. Adverse events matched placebo at both doses. The Cochrane review rates the dementia benefit small to moderate and low certainty, and found little or no effect in mild cognitive impairment (PMID 41641880). In older people with normal cognition, 240 mg a day for about 6 years did not prevent dementia (PMID 19017911). No cited trial tested more than 240 mg.
The dose points come from one randomised, double-blind trial of 78 Japanese adults aged 50 to 69 with memory complaints (PMID 21103034), who took 100 mg or 300 mg a day of soy-derived phosphatidylserine or placebo for 6 months. Test scores rose similarly in all groups, placebo included, so neither dose is shown to beat placebo. Memory scores rose from baseline on both doses only in people with low baseline scores, a within-group result. No side effects and no change in blood markers were seen, which is why the ceiling is 300 mg. The PS-DHA memory trial (PMID 20523044, 157 older adults) states no dose, the 190-person trial in mild cognitive impairment (PMID 39317299) gave 63 mg inside a multi-ingredient capsule, and the pooled 200 to 300 mg effect on inattention (PMID 33539192) is in children with ADHD, so none of them can sit on this curve.
